The SCOFF Questionnaire is a validated assessment tool originally devised by John F. Morgan, Fiona Reid, and J. Hubert Lacey in 1999 in the United Kingdom (UK). It has since generated international validation having been translated into many languages worldwide (Hill et.al., 2010).
The SCOFF remains a key "screening instrument for detecting eating disorders; it is simple, memorable, and easy to apply and score, and it is designed to raise suspicion of a likely case rather than to diagnose" (Morgan, et. al., 1999). If there are two or more affirmative responses, then the person is considered to screen positive for a possible eating disorder, and further assessment is warranted (AED, 2016 & 2021; Luck, et. al., 2002; Morgan, et. al., 1999).
Originally designed for use in primary care settings by physicians in the UK, this self-report measure is a useful self-assessment tool for individuals and/or loved ones of individuals who suspect, or are concerned that they might have some disordered eating behaviors. If one's suspicions appear to be confirmed upon answering these questions, then it only means further evaluation is required. Eating disorders and sub-threshold disordered eating require an in-depth clinical interview and collaborative assessment to determine precisely where on the eating disorder spectrum one's behavior falls and what degree of intervention may or may not be required.
